Information for Blue Cross Blue Shield New Members That Have Not Received Their New ID Cards

Many of our Blue Cross Blue Shield clients have not received their new ID cards due to how slowly Blue Cross Blue Shield is currently processing all applications.

Providers will need the following information to contact Blue Cross Blue Shield to verify membership and eligibility for services:

  • Subscriber name
  • Patient name
  • Patient date of birth
  • Patient mailing address
  • Last 4 digits of the patient SSN
  • Effective date of coverage
